Please provide the legit and valid document during the registration process of the applicant and deceased dies from Covid-19(Corona). List of Documents to be enclosed 1) Aadhaar card of both side of the applicant. 2) Aadhaar card of both side of deceased. 3) Address proof of the applicant issued by competent authority. 4) Address proof of deceased issued by competent authority. 5) Covid-19(Corona) positive certificate of deceased. 6) Death certificate of the deceased issued as per the guidline by the Directorate of Health Services, Govt. of Manipur wherein the reason of death should be mentioned as Covid-19(Corona). 7) Bank Passbook of the legal heir in whose name the assistance to be given. 8) If there is more than one heir, submit affidavit with consent of other heirs.
